A person with chronic kidney disease (CKD) must assimilate a lot of information about the disease. Many aspects that you should take into account. Many decisions to make, many changes to assume. For this reason, we put all our knowledge and heart into ensuring that the passage through our association is as peaceful and calm as possible.
First of all, he is welcomed by the EXPERT PATIENT where the patient can explain his case and resolve doubts about the disease, the types of dialysis, types of access, kidney functions or other important doubts such as whether CKD affects your sexuality, your relationship, or whether you will be able to travel or lead the same life as before being diagnosed.
This professional will be the one who refers the multidisciplinary services that the user needs, or demands, according to what was explained in the reception session.
The EXPERT PATIENT is a person with CKD trained to perform these functions since, in addition to being able to understand the situation that users are experiencing, they know how to communicate with them assertively, know the access and operation of health services, understands non-verbal language, motivates self-care, helps adherence to treatment, improves knowledge about the disease, establishes a relationship of trust between the entity and users and promotes prevention of other pathologies associated with CKD.

- Make sure where you have to go and at what time.
- Take everything they have told you (documents, evidence…).
- Leave early.
- Bring something to eat and drink in case it gets late.
- Take your next medication intake in case you have to stay longer than planned.
In the waiting room
- Manage your nerves.

When leaving the consultation
- Create reminder notes, if necessary, to note what you should bring to the next appointment.
- Write down the date and time of the next appointment in your agenda/calendar/computer.
- Be self-sufficient and independent to manage all the appointment management of your illness.
- Thank the person who accompanied you.
- Smile with satisfaction for having overcome your fears and achieving the challenges set.
Access to health
-
If you still do not have the Health Card that gives you access to health in the Valencian Community, you must call your Primary Care Center and make an appointment so they can issue it to you. In this process, they will surely ask you for the Registration Certificate (requested at the Town Hall or assigned Mayor’s Office) and an identification document such as the NIF.
- List of health centers by district
- Request for free choice of doctor or specialized care service
- Directory of Patient Care and Information Services (SAIP)
- Directory of patient associations of Castellón
- Patient Portal< /a>
It must be taken into account that Primary Care Centers are assigned according to the user’s address. You should know which Health Center is assigned to yours.
In the case of migrants, to apply for the Health Card (SIP) you must be registered in the same home for at least three consecutive months. The documentation that will be required will be the same: the Registration Certificate and an identification document such as a passport.
To obtain more specific information, you must make an appointment with the Social Worker of the Health Center to which you belong by area.
If you do not have a place to register, you can contact us so we can help you in the search. We leave you some links of interest:
